Frimley Hospital Trust is encouraging Young People and their families to complete the Hospital Passport.

This is to support their stay in hospital and hopefully make it a little easier. This can be used / be helpful for anyone even outside of the Frimley Hospital Trust area.

Families can also ask for the ‘Reasonable Adjustment Flag’ which can be added to their NHS profile.
